Summary

Fecal calprotectin (FCAL) testing identified inflammatory bowel disease (IBD) in 17.1% of irritable bowel syndrome (IBS) patients with food intolerances. Dietary changes and treatment normalized FCAL levels in most patients.

Purpose of the Study:

  • To assess the prevalence of elevated FCAL in IBS patients with diagnosed fructose malabsorption (FM), histamine intolerance (HIT), lactose intolerance (LIT), or Helicobacter pylori (H. pylori) infection.
  • To evaluate the impact of targeted dietary interventions and eradication therapy on FCAL levels in this patient cohort.

Main Methods:

  • Retrospective analysis of 228 patients diagnosed with IBS spectrum disorders due to food intolerances/malabsorption or H. pylori infection.
  • FCAL levels were measured to screen for IBD.
  • Patients with elevated FCAL underwent further evaluation, including colonoscopy and biopsy when indicated. Follow-up FCAL measurements were obtained after dietary and/or therapeutic interventions.

Main Results:

  • Elevated FCAL levels were detected in 39 (17.1%) of the 228 IBS patients.
  • Lactose intolerance was the most frequent condition associated with elevated FCAL, followed by fructose malabsorption, histamine intolerance, and H. pylori infection, with various combinations present.

  • FCAL testing can be a valuable tool in the diagnostic workup of IBS patients with suspected food intolerances or malabsorption, aiding in the identification of organic IBD.
  • Dietary management and eradication therapy are effective in reducing elevated FCAL levels in patients with functional gastrointestinal disorders, suggesting a link between gut inflammation and these conditions.
  • This study highlights the importance of considering food intolerances and infections as potential drivers of elevated FCAL, necessitating a comprehensive diagnostic approach in IBS patients.
