Blood and Nerve Supply to the Bones01:29

Blood and Nerve Supply to the Bones

11.4K
Bones are dynamic organs that require a rich supply of oxygen and nutrients. Around 5% to 10% of the cardiac output supplies blood to the bones. A typical long bone has three main sources: the nutrient artery, the metaphyseal and epiphyseal arteries, and the periosteal arteries.
Nutrient Artery
The nutrient artery is the main blood vessel that enters the diaphysis via the nutrient foramen. While most long bones have only one nutrient foramen, large bones, such as the femur, may have two. This...

Nociception

28.2K
Nociception—the ability to feel pain—is essential for an organism’s survival and overall well-being. Noxious stimuli such as piercing pain from a sharp object, heat from an open flame, or contact with corrosive chemicals are first detected by sensory receptors, called nociceptors, located on nerve endings. Nociceptors express ion channels that convert noxious stimuli into electrical signals. When these signals reach the brain via sensory neurons, they are perceived as pain.

Area of Science:

  • Infectious Diseases
  • Pain Management
  • Public Health

Background:

  • The 2022 monkeypox virus (MPXV) outbreak raised concerns, with severe pain as a distressing complication.
  • Limited peer-reviewed literature exists for effective MPXV pain management strategies.
  • Urgent need for research on pain control in MPXV patients was highlighted by the CDC.

Purpose of the Study:

  • To conduct a rapid review of existing literature on pain management for monkeypox virus (MPXV).
  • To summarize proposed analgesic treatments for MPXV-associated pain.
  • To identify gaps in current knowledge and guide future research.

Main Methods:

  • A systematic literature search was performed following PRISMA guidelines.
  • Databases were searched from inception until August 19, 2022, for English-language studies.
  • Studies with original data on pain in MPXV-positive patients were included; 15 studies comprising 1043 cases were analyzed.

Main Results:

  • Most MPXV patients were male; severe pain was a common reason for seeking medical care and hospitalization.
  • Proposed treatments included acetaminophen, ibuprofen, opioids, topical lidocaine, metamizole, and rectal therapies for severe anal pain.
  • Oral and topical analgesics were common; opioids were used for severe pain management.

Conclusions:

  • This is the first comprehensive summary of proposed MPXV pain treatments.
  • Guidelines are needed to direct optimal pain management and reduce patient morbidity.
  • Adjuvant therapies may be crucial but are underutilized in reported cases.
