Background:

  • Gamma knife (GK) thalamotomy is an established treatment for essential tremor (ET).
  • Existing research indicates variability in treatment responses and complication rates for GK thalamotomy in ET patients.

Purpose of the Study:

  • To evaluate the efficacy and safety of Gamma knife thalamotomy for essential tremor.
  • To assess the impact of GK thalamotomy on tremor severity, handwriting, and drawing tasks in ET patients.

Main Methods:

  • Retrospective analysis of 27 patients with essential tremor who underwent GK thalamotomy.
  • Evaluation using the Fahn-Tolosa-Marin Clinical Rating Scale for Tremor, handwriting, and spiral drawing.
  • Assessment of postoperative adverse events and magnetic resonance imaging findings.

Main Results:

  • Significant improvements were observed in postural tremor (55.9%), handwriting (57.6%), and spiral drawing (50%) scores post-GK thalamotomy (P < 0.001).
  • Three patients showed no tremor improvement; six experienced adverse effects, including hemiparesis and numbness.
  • Two patients had serious complications (hemiparesis, hematoma), and one patient died from aspiration pneumonia secondary to dysphagia.

Conclusions:

  • Gamma knife thalamotomy is an effective procedure for essential tremor management.
  • Careful treatment planning is essential to mitigate complication risks.
  • Predicting radiation complications can enhance the safety and effectiveness of GK treatment for ET.
