Background:

High offset liners are utilized in total hip arthroplasty (THA) to restore soft tissue tension.
Previous studies indicated a potential increase in aseptic loosening with offset components.

Purpose of the Study:

To compare postoperative complication and revision rates between neutral and offset acetabular liners in primary THA.
To evaluate the safety and efficacy of contemporary high offset liners.

Main Methods:

Retrospective review of 208 primary THAs (206 patients) using neutral or offset liners (4 mm thickness).
Minimum 2-year clinical follow-up (mean 3.3 years).
Analysis of postoperative complications, aseptic loosening, and revision surgeries.

Main Results:

No significant difference in complication rates between neutral (10.0%) and offset (14.8%) groups (P > .05).
No aseptic loosening reported in the offset group versus two cases in the neutral group.
Revision rates were 7.5% for the neutral group and 3.6% for the offset group, with no significant difference (P > .05).

Contemporary high offset liners (up to 4 mm) are safe and effective in primary THA.
These liners do not increase the risk of postoperative complications or revision surgery compared to neutral liners.
Offset liners may help restore native hip mechanics without compromising short-term outcomes.
