Background:

Extragastrointestinal stromal tumors (EGISTs) originate outside the gastrointestinal tract, commonly in the omentum, mesentery, or retroperitoneum.
Perianal EGISTs represent an exceedingly rare clinical presentation.

Observation:

A 70-year-old male presented with a two-year history of anal discomfort.
Pelvic MRI revealed a homogenous, well-defined soft tissue mass.
Surgical excision and subsequent pathological examination confirmed the mass as an EGIST.

Findings:

The perianal EGIST measured 4.3×3.2 cm with a low mitotic count (1/50 HPF).
Immunohistochemistry showed positivity for KIT and CD34, with negativity for S-100 and alpha-smooth muscle actin.
No other gastrointestinal tract abnormalities were identified.

Implications:

EGISTs must be considered in the differential diagnosis of perianal masses.
This case underscores the importance of comprehensive histopathological evaluation for rare perianal tumors.
Awareness of EGISTs in this location can improve diagnostic accuracy and patient management.
