Background:

  • Extragastrointestinal stromal tumors (EGISTs) originate outside the gastrointestinal tract, commonly in the omentum, mesentery, or retroperitoneum.
  • Perianal EGISTs represent an exceedingly rare clinical presentation.

Observation:

  • A 70-year-old male presented with a two-year history of anal discomfort.
  • Pelvic MRI revealed a homogenous, well-defined soft tissue mass.
  • Surgical excision and subsequent pathological examination confirmed the mass as an EGIST.

Findings:

  • The perianal EGIST measured 4.3×3.2 cm with a low mitotic count (1/50 HPF).
  • Immunohistochemistry showed positivity for KIT and CD34, with negativity for S-100 and alpha-smooth muscle actin.
  • No other gastrointestinal tract abnormalities were identified.

Implications:

  • EGISTs must be considered in the differential diagnosis of perianal masses.
  • This case underscores the importance of comprehensive histopathological evaluation for rare perianal tumors.
  • Awareness of EGISTs in this location can improve diagnostic accuracy and patient management.
