This study presents a surgical strategy for combining nasal fracture reduction with cosmetic rhinoplasty. The approach successfully improved both functional and aesthetic outcomes in patients with pre-existing nasal deformities.

Background:

  • Simultaneous reduction of nasal bone fractures and cosmetic rhinoplasty presents significant surgical challenges due to fracture severity and skeletal instability.

Purpose of the Study:

  • To investigate a surgical strategy and evaluate outcomes for cosmetic rhinoplasty performed concurrently with nasal bone fracture reduction.
  • To categorize and address pre-existing nasal deformities within the context of combined fracture repair and aesthetic surgery.

Main Methods:

  • A cohort of 71 patients undergoing cosmetic rhinoplasty was analyzed.
  • Patients were categorized by pre-existing deformities (dorsal hump, irregularities, deviation, nasal width, flat dorsum).
  • Individualized surgical maneuvers using autologous grafts were employed, followed by photoanalysis and subjective outcome evaluation.

Main Results:

  • Photoanalysis revealed significant improvements in nasal parameters (deviation, length, radix height, dorsal height, tip projection, angles) (P <0.05).
  • Patient satisfaction scores were significantly higher postoperatively (P <0.05).
  • Minor complications included two cases of tip deviation due to septal instability, successfully revised.

Conclusions:

  • The developed algorithm for evaluating, categorizing, and treating nasal deformities in fracture patients allows for harmonious facial reconstruction.
  • Simultaneous nasal fracture reduction and cosmetic rhinoplasty, when tailored to individual deformities, yield favorable functional and aesthetic results.
