Primary cardiac lymphoma (PCL) is a rare heart tumor. F18-fluorodeoxyglucose positron emission tomography (18 FDG-PET) aids in diagnosing PCL presenting as pyrexia of unknown origin (PUO).

Background:
- Primary cardiac lymphoma (PCL) is an exceptionally rare malignancy, accounting for only 1% of all cardiac tumors.
- PCL often affects the right side of the heart, and its diagnosis is frequently delayed due to vague symptoms and challenging localization.
Observation:
- A middle-aged male presented with pyrexia of unknown origin (PUO).
- Diagnostic evaluation, including F18-fluorodeoxyglucose positron emission tomography (18 FDG-PET), revealed PCL.
Findings:
- 18 FDG-PET imaging proved crucial in identifying the cardiac lesion.
- PET-CT effectively localized the tumor, facilitating prompt tissue diagnosis and intervention planning.
Implications:
- This case highlights the utility of PET-CT in diagnosing neoplasms presenting as PUO.
- Physicians should consider PCL in the differential diagnosis of PUO, especially when symptoms mimic common cardiac tumors like atrial myxoma.
