    Summary

    This study describes a rare case of bilateral chronic paroxysmal hemicrania (CPH), a headache syndrome usually presenting as strictly one-sided attacks. This finding expands the known clinical spectrum of CPH.

    • A patient with a probable bilateral presentation of chronic paroxysmal hemicrania (CPH) is presented.

    Findings:

    • This case represents a probable bilateral CPH.
    • No prior documented cases of bilateral CPH exist in medical literature.

    Implications:

    • This case expands the diagnostic understanding of chronic paroxysmal hemicrania.
    • Further research may explore the underlying mechanisms and prevalence of bilateral CPH.
