    Background:

    • Plateletpheresis is a critical procedure for collecting platelets for transfusion.
    • Various apheresis devices and protocols exist, each with potential differences in efficiency and safety.

    Purpose of the Study:

    • To compare the efficacy and safety of six different plateletpheresis protocols.
    • To evaluate platelet yield and leukocyte contamination across different apheresis systems.

    Main Methods:

    • Seven hundred ten plateletpheresis procedures were analyzed.
    • Six distinct protocols were employed, utilizing Haemonetics 30, Fenwal CS-3000, IBM 2997 (dual and single-stage channels), and Haemonetics V-50 surge pump methods.
    • Anticoagulant solutions (ACD-A, ACD-B) were also assessed with the IBM 2997 single-stage channel.

    • Haemonetics 30 and IBM 2997 single-stage channel yielded high platelet counts (5.4-5.9 X 10^11) but significant leukocyte contamination (10.8-11.6 X 10^9).
    • Fenwal CS-3000, IBM 2997 dual-stage, and Haemonetics V-50 surge pump showed lower leukocyte contamination (0.3-0.9 X 10^9) with reduced platelet yield (3.4-4.3 X 10^11).
    • No significant difference in yield was noted between ACD-A and ACD-B with the IBM 2997 single-stage channel. Paresthesias was the most common side effect, more frequent with ACD-A and IBM 2997.

    Conclusions:

    • Apheresis device selection influences both platelet yield and the level of leukocyte contamination in platelet concentrates.
    • While some protocols maximize platelet collection, they may compromise purity; others offer cleaner products at the expense of yield.
    • Donor side effects, such as paresthesias, are associated with specific anticoagulant and device combinations.
