    Summary

    Blunt trauma can cause traumatic aneurysms in the superficial femoral artery (SFA) and superior medial geniculate artery (SMGA). These pulsatile hematomas can expand, leading to limb ischemia and requiring surgical repair.

    Background:

    • Traumatic aneurysms, particularly pseudoaneurysms, can arise from blunt force injuries to major arteries.
    • The superficial femoral artery (SFA) and superior medial geniculate artery (SMGA) are susceptible to such injuries, potentially leading to significant complications.

    Observation:

    • Late expansion of previously contained hematomas in the SFA and SMGA was observed.
    • These expanding hematomas were pulsatile and associated with audible bruits.
    • Injury to the SFA specifically caused a cool, pulseless distal extremity, indicating compromised blood flow.

    Findings:

    • Direct surgical exploration and repair of the arterial defect were successfully performed after achieving proximal and distal control of the SFA.
    • Blunt trauma is confirmed as a cause of pseudoaneurysm formation in these arteries.
    • Persistent swelling at the injury site, even with normal distal pulses, serves as a critical early warning sign for major vascular injury.

    Implications:

    • Prompt recognition and surgical intervention are crucial for managing traumatic pseudoaneurysms to prevent limb-threatening complications.
    • Enhanced vigilance for vascular injury is warranted in patients presenting with localized swelling post-blunt trauma.
    • This case highlights the importance of considering vascular complications in the assessment of lower extremity trauma.
