Purpose of the Study:

  • To evaluate racial and ethnic disparities in postmastectomy breast reconstruction.
  • To compare rates of discussion, referrals, and decisions regarding breast reconstruction across different racial/ethnic groups.

Main Methods:

  • A review of all women undergoing mastectomy for breast cancer at a single institution between 2017 and 2018.
  • Comparison of reconstruction discussion, plastic surgery referrals, consultations, and decisions by race/ethnicity.

Main Results:

  • Overall breast reconstruction incidence was 48%, significantly lower in Black women (34%) compared to white women (58%).
  • Discussion and referral rates for plastic surgery did not vary by race/ethnicity but were lower with interpreter need.
  • Lower reconstruction rates were associated with Black race (OR=0.33) and high BMI (OR=0.14), independent of each other.

Conclusions:

  • Black women have significantly lower rates of breast reconstruction than white women, even with equivalent discussion and referral rates.
  • Barriers to care likely contribute to these observed racial disparities in breast reconstruction.
  • Further community-based exploration is necessary to understand and address the racial disparity in breast reconstruction outcomes.
