Summary

Cervical cerclage rarely causes serious complications like fistula formation, especially vesicovaginal fistulas. This study investigated the incidence and factors associated with these rare but serious complications.

Purpose of the Study:

  • To evaluate the incidence of fistula formation or erosion after transvaginal cervical cerclage.
  • To identify associated clinical and sociodemographic factors.

Main Methods:

  • Systematic literature search of PubMed, Medline, and Embase databases up to July 2021.
  • Inclusion of 9 full-text articles (7 case reports/series) and 2 retrospective reviews.
  • Analysis of 11 patients with late complications and 75 patients from reviews.

Main Results:

  • Fistula formation was the most common late complication, particularly vesicovaginal fistulas (63.6%).
  • Overall fistula incidence was 1.3% in retrospective reviews.
  • Elective procedures (66.7%) and McDonald cerclage (80%) were common.

Conclusions:

  • Fistula formation, especially vesicovaginal, is the most common rare long-term complication of cervical cerclage.
  • Further research is needed to clarify risk factors for these uncommon but serious complications.
