Background:

  • Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is an uncommon hematologic malignancy.
  • It originates from plasmacytoid dendritic cell precursors (pDCs) and is characterized by an aggressive clinical course.
  • The disease often presents initially with skin lesions before potential extracutaneous involvement.

Observation:

  • A case report of a 72-year-old male with painless skin lesions on the left anterior chest wall.
  • Histological examination revealed diffuse dermal infiltration by medium-sized blastic cells.
  • Immunophenotypic analysis showed positivity for CD4, CD45, CD7, CD56, CD43, CD123, TCL1, and BCL2.

Findings:

  • The immunophenotype is crucial for diagnosing BPDCN.
  • The observed blastic cells exhibited a specific marker profile consistent with BPDCN.
  • The initial presentation involved localized skin lesions.

Implications:

  • BPDCN diagnosis relies heavily on characteristic immunophenotypic markers.
  • Adapted chemotherapy regimens from leukemia and lymphoma treatments are utilized due to the rarity of BPDCN.
  • This case underscores the importance of recognizing cutaneous manifestations for early BPDCN detection and management.
