    Summary

    Mesencephalic hematomas, often presenting with headache and vomiting, can cause consciousness impairment and abnormal pupillary responses. These hematomas, identified via CT, require careful diagnosis to rule out other serious conditions.

    Background:

    • Mesencephalic hematomas are rare intracranial hemorrhages.
    • Understanding their clinical and radiological presentation is crucial for diagnosis and management.

    Observation:

    • Six patients with mesencephalic hematomas were analyzed for clinical and CT findings.
    • Common symptoms included headache, vomiting, impaired consciousness, unequal pupils with poor light reaction, and impaired upgaze.

    Findings:

    • CT scans revealed unilateral, nonenhancing midbrain hematomas in all patients.
    • Intraventricular blood was present in five patients.
    • Four-vessel angiography excluded neoplasms, angiomas, or aneurysms as the cause.
