Herniated Intervertebral Disc l: Introduction01:29

Herniated Intervertebral Disc l: Introduction

Intervertebral disc herniation refers to the displacement of the nucleus pulposus (the gel-like inner core of the disc) through a tear or weakened area in the annulus fibrosus (the outer fibrous ring). The displaced disc material extends beyond the normal boundaries of the disc space and may compress or irritate nearby spinal nerve roots or, less commonly, the spinal cord.Etiology and Risk FactorsHerniation commonly results from degeneration, in which aging reduces disc hydration and...
Degenerative Disc Disease I: Introduction01:27

Degenerative Disc Disease I: Introduction

Degenerative disc disease is a chronic condition in which intervertebral discs gradually lose structure and function. It is not infectious or autoimmune; rather, it results from age-related biochemical and mechanical changes, influenced by genetic, metabolic, and environmental factors.Structure and Function of DiscsThe spine contains 23 intervertebral discs that absorb load, distribute forces, maintain spacing, and allow flexibility. Each disc consists of a nucleus pulposus, a gel-like core...

Summary

Dynamic Stabilization Systems (DSS) may promote disc rehydration in patients with lumbar degenerative disease (LDD). This review found a 27.6% rehydration rate, suggesting DSS could delay LDD progression.

Area of Science:

  • Spine Surgery
  • Biomedical Engineering
  • Regenerative Medicine

Background:

  • Lumbar degenerative disease (LDD) treatment traditionally uses fusion with posterior screw fixation, but complications are rising.
  • Dynamic Stabilization Systems (DSS) offer an alternative for LDD, though clinical outcomes require further investigation.
  • This study systematically reviews the potential of DSS to promote intervertebral disc rehydration in LDD patients.

Approach:

  • A systematic review of studies published before June 2022 from Pubmed, Web of Science, and Embase databases.
  • Keywords included 'rehydration', 'lumbar', 'degenerative disc disease', and 'Dynamic Stabilization Systems'.
  • Quality assessment used the Newcastle-Ottawa Scale and Joanna Briggs Institute critical appraisal checklist.

Key Points:

  • Seven studies (5 case series, 2 case-control) involving 199 patients were included.
  • Disc rehydration was observed in 55 cases (27.6%), assessed by Pfrimann grading on MRI.
  • Dynamic Stabilization Systems showed positive clinical outcomes, with significant improvements in pain and function scores.

Conclusions:

  • Dynamic Stabilization Systems may facilitate disc rehydration and potentially slow the progression of lumbar degenerative disease.
  • Mechanical stretch appears to be a key factor in intervertebral disc rehydration.
  • Findings support the clinical application of Dynamic Stabilization Systems for lumbar degenerative disease.
