Summary

Post-exposure prophylaxis (PEP) for COVID-19, including hydroxychloroquine, showed no established clinical benefits. Further research is needed to explore potential therapeutic effects of certain agents against SARS-CoV-2 infection.

Background:

  • Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) causes pneumonia and can lead to acute respiratory distress syndrome (ARDS).
  • Post-exposure prophylaxis (PEP) is a strategy to prevent viral transmission, but its efficacy for COVID-19 remains inconclusive.
  • Understanding the potential of PEP is crucial for managing SARS-CoV-2 infections and preventing severe outcomes.

Approach:

  • A systematic review was conducted to evaluate studies on post-exposure prophylaxis (PEP) for COVID-19.
  • Literature search was performed on major databases (Cochrane, PubMed, Web of Science, Scopus) from December 2019 to August 2021.
  • Included studies were screened using a two-phase process, adhering to PRISMA guidelines.

Key Points:

  • Hydroxychloroquine (400-800 mg daily for 5-14 days) was the most frequently studied agent for COVID-19 PEP.
  • Other agents investigated included Chloroquine, Lopinavir-ritonavir (LPV/r), ACEIs, ARBs, Vitamin D, arbidol, thymosin, and Xin guan no.1 (XG.1).
  • A total of 17 resources met the inclusion criteria for the systematic review.

Conclusions:

  • Current evidence indicates no established clinical benefits for any drug used as PEP in individuals with COVID-19.
  • Some agents showed scarce indications of beneficial effects, warranting further investigation.
  • More research is necessary to definitively determine the efficacy and safety of PEP for COVID-19.
