Summary

Malignant glaucoma is a rare complication after gonioscopy-assisted transluminal trabeculotomy (GATT). Prompt medical therapy and YAG hyaloidotomy successfully resolved this sight-threatening condition.

Background:

Pseudoexfoliation glaucoma and open-angle glaucoma are common causes of secondary glaucoma.
Gonioscopy-assisted transluminal trabeculotomy (GATT) is an effective surgical option for managing refractory glaucomas.

Observation:

A 69-year-old male with pseudoexfoliation and open-angle glaucoma developed malignant glaucoma one week post-GATT.
This represents a rare and sight-threatening complication following GATT procedures.

Findings:

Early detection and a high index of suspicion were crucial for diagnosis.
Prompt medical management combined with YAG hyaloidotomy led to successful resolution.
The patient achieved good intraocular pressure control and visual improvement.

Implications:

Highlights the importance of vigilance for malignant glaucoma after GATT.
Suggests a multimodal treatment approach for managing this complication.
Emphasizes the potential for favorable outcomes with timely intervention in malignant glaucoma post-GATT.
