    Summary

    A patch test for Kathon CG (a preservative) showed a low positive reaction rate. However, subsequent use tests indicated that a positive patch test does not necessarily lead to eczema from products containing this common cosmetic ingredient.

    Background:

    • Kathon CG is a widely used preservative in cosmetic and personal care products.
    • Concerns exist regarding its potential to cause allergic contact dermatitis.

    Purpose of the Study:

    • To evaluate the clinical relevance of patch test reactions to Kathon CG.
    • To determine if positive patch test results predict eczema development from product use.

    Main Methods:

    • 1511 patients underwent patch testing with Kathon CG at 100 ppm.
    • 11 patients with positive patch tests were subsequently tested with a lotion containing 8.6 ppm Kathon CG.

    Main Results:

    • A positive patch test reaction to Kathon CG (100 ppm) was observed in 13 patients (0.8%).
    • None of the 11 patients with a positive patch test developed eczema upon using the Kathon CG-containing lotion (8.6 ppm).

    Conclusions:

    • A positive patch test reaction to Kathon CG at 100 ppm does not reliably predict the development of eczema from exposure to lower concentrations (3-15 ppm) found in final consumer products.
    • The clinical significance of a positive patch test to Kathon CG may be limited for typical product use concentrations.
