Imaging changes following surgery for ischiofemoral impingement

Partial resection of the lesser trochanter (LT plasty) for ischiofemoral impingement leads to a significant decrease in iliopsoas muscle size. This reduction in muscle size was not correlated with midterm functional outcomes after surgery.

Background:
- Ischiofemoral impingement (IFI) is a condition characterized by hip pain, often treated with partial resection of the lesser trochanter (LT plasty).
- Limited research exists on the post-operative imaging findings following LT plasty for IFI.
Purpose of the Study:
- To evaluate magnetic resonance imaging (MRI) changes in the lesser trochanter and surrounding hip musculature after LT plasty for IFI.

- Twenty-one patients undergoing LT plasty for IFI were included.
- MRI scans were performed pre- and post-operatively (average 11 months post-op).
- Lesser trochanter length and iliopsoas muscle cross-sectional area were measured and compared.

- The average amount of lesser trochanter resected was 7.3 mm.
- Iliopsoas muscle cross-sectional area decreased by an average of 35% in 95% of patients post-surgery.
- The reduction in iliopsoas size did not correlate with modified Harris Hip Score improvements at midterm follow-up.
Conclusions:
- Endoscopic LT plasty results in an average 35% decrease in iliopsoas muscle size.
- This muscle size reduction is not associated with midterm functional outcomes in patients with ischiofemoral impingement.
