Summary

The novel cinnamon roll technique using wide-awake local anesthesia no tourniquet (WALANT) effectively reduces nipple hypertrophy. This safe and simple method allows patients to achieve their desired nipple size with high satisfaction.

Background:

Purpose of the Study:

  • To introduce and evaluate a novel "cinnamon roll" technique for nipple reduction.
  • To assess the efficacy and patient satisfaction of the "cinnamon roll" technique performed under wide-awake local anesthesia no tourniquet (WALANT).

Main Methods:

  • Fifteen patients (30 nipples) underwent nipple reduction using the "cinnamon roll" technique with WALANT between November 2015 and October 2022.
  • Nipple dimensions, pain scores (VAS), and aesthetic satisfaction were recorded.
  • Sensory recovery was monitored for up to 12 months post-surgery.

Main Results:

  • Pre-operative mean nipple diameter and height were 13.2 mm and 12 mm, respectively; post-operative measurements reduced to 8.8 mm and 8.7 mm.

  • No significant complications like necrosis, infection, or hypertrophic scarring were reported.

Conclusions:

  • The "cinnamon roll" technique combined with WALANT is a safe, reliable, and easily learned procedure for nipple reduction.
  • This technique empowers patients to actively participate in achieving their ideal nipple size, leading to high satisfaction rates.
