A pilot study found that a stretching program was more effective than app-based cognitive behavioral therapy (CBT) for reducing pediatric migraine disability. Quality of life did not significantly differ between groups.
Area of Science:
- Pediatric Neurology
- Digital Health Interventions
- Behavioral Therapy
Background:
- Pediatric headaches, particularly migraines, significantly impact quality of life.
- Cognitive Behavioral Therapy (CBT) is a recognized treatment for headaches, but its app-based delivery in pediatrics requires further investigation.
- Comparing digital interventions with traditional approaches like stretching is crucial for understanding optimal treatment strategies.
Purpose of the Study:
- To compare the efficacy and feasibility of app-based CBT versus a stretching program in pediatric headache patients.
- To assess the impact of these interventions on headache-related disability and quality of life.
- To explore group effects while controlling for adherence and baseline scores.
Main Methods:
- A pilot randomized-controlled 4-week trial.
- Inclusion of pediatric patients with headaches.
- Assessment using Pediatric Migraine Disability Scale (PedMIDAS), Kidscree27, and Pediatric Quality of Life Inventory.
- Multivariable regression analysis to evaluate group effects and adherence.
Main Results:
- Higher adherence was observed in the stretching group (100%) compared to the CBT app group (54%).
- The stretching group demonstrated a significantly greater reduction in PedMIDAS scores (P < .05) after controlling for adherence and baseline.
- No significant differences were found between groups in quality-of-life indicators (P > .05).
Conclusions:
- App-based CBT was not superior to a stretching program in reducing headache-related disability in this pediatric population.
- Stretching may be a viable alternative or adjunct therapy for pediatric headache management.
- Future research should consider optimizing CBT app features for pediatric users to potentially enhance outcomes.
