    Background:

    • Trichoepitheliomas are benign tumors of follicular germ cells.
    • They typically present as multiple papules and nodules on the face.
    • Genetic mutations in CYL1, GLI2, or KRT17 are associated with trichoepitheliomas.

    Observation:

    • A 10-year-old girl presented with multiple trichoepitheliomas.
    • Lesions initially appeared on the neck and later involved the face, trunk, and lower extremity.
    • A unique linear and dermatomal distribution was noted on the lower extremity.

    Findings:

    • This is the first reported case of trichoepitheliomas exhibiting a linear and dermatomal pattern.
    • The presentation suggests a possible mosaic form of trichoepithelioma or a novel manifestation.
    • Histopathological confirmation of trichoepitheliomas was established.

    Implications:

    • This case expands the known spectrum of trichoepithelioma presentations.
    • It may prompt further investigation into the genetic and developmental mechanisms underlying linear and dermatomal skin conditions.
    • Understanding this rare presentation can aid in accurate diagnosis and management of similar cases.
