This study reports a rare bladder tumor diagnosed as carcinosarcoma in a 78-year-old man. The tumor showed sarcomatous changes evolving from squamous metaplasia of transitional cell carcinoma.

    Area of Science:

    • Uropathology
    • Oncology
    • Surgical Pathology

    Background:

    • Bladder tumors can present with complex histopathological features.
    • Carcinosarcoma is a rare malignant neoplasm characterized by the simultaneous presence of carcinomatous and sarcomatous elements.

    Observation:

    • A 78-year-old male patient presented with two independent, solid pedunculated bladder tumors.
    • Histopathological examination revealed predominantly spindle-shaped sarcomatous tissue with multinucleated giant cells.
    • Transitional cell carcinoma (Grade 3) with squamous metaplasia was identified at the tumor stalk.

    Findings:

    • The sarcomatous component appeared to arise from squamous metaplasia of the transitional cell carcinoma.
    • No direct transition from transitional cell carcinoma to sarcomatous tissue was observed.

    Related Experiment Videos

  • The tumor was diagnosed as a carcinosarcoma, indicating a biphasic malignant tumor.
  • Implications:

    • This case highlights the potential for sarcomatous differentiation within bladder tumors via squamous metaplasia.
    • Understanding the pathway of sarcomatous change is crucial for accurate diagnosis and treatment of carcinosarcoma.
    • Further research into the pathogenesis of carcinosarcoma may improve patient outcomes.
