    Background:

    • Chronic iliac vein obstruction is a cause of venous claudication.
    • The underlying mechanisms of pain in this condition are not fully understood.

    Purpose of the Study:

    • To investigate the relationship between chronic iliac vein obstruction and tissue pressures.
    • To understand the pathophysiology of pain in venous claudication.

    Main Methods:

    • Ten patients with unilateral post-thrombotic syndrome and chronic iliac vein obstruction were studied.
    • Subcutaneous and intramuscular pressures were measured using the slit-catheter technique in both legs.
    • Measurements were taken at rest and during exercise in supine and standing positions.

    • At rest, subcutaneous tissue pressure was slightly higher in the affected leg, but not significantly.

    Conclusions:

    • Elevated intramuscular pressures during exercise may contribute to venous claudication pain in chronic iliac vein obstruction.
    • The findings highlight the impact of venous obstruction on limb hemodynamics and pain generation.
