Background:

  • Primary retroperitoneal tumors are uncommon, with primary retroperitoneal serous carcinoma being exceptionally rare.
  • Only 15 cases of this specific tumor type have been previously documented in medical literature.

Observation:

  • A 62-year-old female patient presented with symptoms of abdominal bloating.
  • Positron Emission Tomography/Computed Tomography (PET/CT) revealed two distinct tumors with elevated Fluorodeoxyglucose (FDG) uptake near the right common iliac vessels.
  • Surgical exploration identified no ascites but did reveal a myomatous uterus within the peritoneal cavity.

Findings:

  • Surgical resection of the retroperitoneal tumors was performed.
  • The patient received adjuvant chemotherapy consisting of paclitaxel and carboplatin.
  • The patient experienced a favorable outcome, remaining disease-free for 80 months post-treatment.

Implications:

  • This case underscores the diagnostic value of 18F-FDG PET/CT in characterizing indeterminate retroperitoneal masses.
  • Early and accurate diagnosis is critical for effective management of rare retroperitoneal malignancies.
  • The successful outcome suggests that a multimodal approach including surgery and chemotherapy can be effective for this rare condition.
