    Background:

    Purpose of the Study:

    • To conduct a clinicopathological investigation of massive hepatic necrosis in patients experiencing shock.
    • To identify associated conditions, macroscopic and microscopic pathological features, and potential pathogenetic mechanisms.
    • To evaluate the potential therapeutic role of heparin.

    Main Methods:

    • Clinicopathological analysis of 15 cases with massive hepatic necrosis post-shock.
    • Assessment of liver function tests (GOT and GPT levels).
    • Autopsy examination including liver weight, macroscopic ('map-like necrosis'), and microscopic (centrilobular necrosis) findings.

    • Elevated GOT and GPT (>1000 units) observed in 10 out of 15 cases.

    • Massive hepatic necrosis in shock likely results from severe systemic or intrahepatic circulatory disturbances.
    • Repeated severe shock, potentially involving Shwartzman reactions and fibrin thrombi, may contribute to pathogenesis.
    • Heparin therapy may be beneficial if administered timely.
