Models of Health Promotion and Illness Prevention I

2.1K
A model is a theoretical way to understand a concept or an idea. Models can overcome barriers to health regardless of diverse economic and cultural backgrounds. In addition, models make the task easier by providing different ways to approach complex issues. There are two major health promotion models: the health belief model and the health promotion model.
The health belief model (HBM) attempts to predict health-related behavior in specific belief patterns. According to the HBM, a person's...

The person's health status fluctuates continually, varying from being in good health to becoming ill and returning to being healthy. To understand the concept of illness prevention, there are two models. First, the health-illness continuum model is a graphic representation of an individual's wellness. It states that a person is considered healthy in the absence of physical disease and the presence of good emotional health.
The agent-host-environment model states that disease results...

Methods of Documentation VI: Case Management Model

606
The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.

Area of Science:

  • Public Health
  • Medical Practice
  • Substance Use Disorders

Background:

  • Substance use disorders (SUDs) present significant public health challenges.
  • Harm reduction offers a strategy to mitigate risks associated with SUDs.
  • Integrating harm reduction into medical care is crucial for patient engagement.

Purpose of the Study:

  • To describe how three medical practices implemented harm reduction.
  • To identify common practices for integrating harm reduction into medical care.
  • To understand the operationalization of harm reduction frameworks.

Main Methods:

  • Qualitative study utilizing in-depth, semistructured interviews.
  • Interviews conducted with 20 staff and providers at three integrated sites.
  • Explored implementation, demonstration, and barriers of harm reduction approaches.

Main Results:

  • Identified 8 main themes of integrated harm reduction medical care.
  • Themes included provider roles, pragmatic success measures, collaborative teams, stigma-free culture, welcoming spaces, low-threshold care, external outreach, and referral networks.
  • Themes spanned patient-provider, organizational, and external levels.

Conclusions:

  • All three sites demonstrated 8 common themes in harm reduction-informed care.
  • These practices align with patient-centered care principles.
  • Medical providers can integrate harm reduction to overcome traditional medical model barriers.
