Impulse-Control Disorders and Restless Leg Syndrome in Parkinson's Disease: Association or Coexistence

Background:

• Impulse-control disorders and related behaviors (ICD-RB) and restless leg syndrome (RLS) are frequent in Parkinson's disease (PD).
• The relationship between ICD-RB and RLS as complications of dopaminergic therapy in PD remains unclear.

Purpose of the Study:

• To investigate the correlation between ICD-RB and RLS in Parkinson's disease patients.
• To identify the psycho-behavioral profile associated with RLS in PD patients who also have ICD-RB.

Main Methods:

• Screened 122 PD patients for ICD-RB using the QUIP questionnaire and RLS using international diagnostic criteria.
• Categorized patients into groups based on the presence or absence of both ICD-RB and RLS.
• Analyzed demographic, socioeconomic, and disease characteristics in relation to RLS and ICD-RB co-occurrence.

Main Results:

• Of 95 eligible PD patients, 53.6% had ICD-RB and 18.9% had RLS.
• Among patients with RLS, 66.7% also had at least one ICD-RB.
• Gambling and compulsive eating were significantly associated with the PD-RLS group. PD patients with both RLS and ICD-RB had longer disease duration and higher daily levodopa equivalent dose (LEDD).

Conclusions:

• Approximately 11% of Parkinson's disease patients experience co-occurring RLS and ICD-RB.
• Hyper-dopaminergic states and circadian dopamine fluctuations may contribute to these co-occurring conditions.
• Long-term dopaminergic treatment or the neurodegenerative process itself might precipitate both RLS and ICD-RB in PD.
