    Background:

    • Isoimmune neonatal neutropenia (INN) is a condition where maternal antibodies target infant neutrophils.
    • The exact incidence and clinical spectrum of INN require further elucidation.

    Purpose of the Study:

    • To determine the incidence of INN in a tertiary perinatal center.
    • To characterize the clinical and immunologic features of INN.

    Main Methods:

    • Screening of 1465 newborn infants for neutropenia.
    • Evaluation of neutropenic infants for antineutrophil antibodies using EDTA-microagglutination and indirect immunofluorescence.

    Main Results:

    • Three cases of INN were confirmed among 16 infants with persistent neutropenia.
    • The incidence of INN was found to be 2 per 1000 live births.
    • INN accounted for 1.5% of special care nursery admissions.

    Conclusions:

    • INN is not a rare disorder and should be considered in all neutropenic infants, irrespective of infection status.
    • Advances in neutrophil-specific antigen systems improve diagnosis and treatment of immune neutropenias.
