Summary

Hepatitis B virus (HBV) reactivation can occur in patients with hematological malignancies treated with ruxolitinib. This case highlights the importance of continuous HBV prophylaxis, even after initial treatment, to prevent reactivation during ruxolitinib therapy.

  • Patients with hematological malignancies and prior hepatitis B virus (HBV) infection are susceptible to HBV reactivation.
  • Ruxolitinib, a JAK 1/2 inhibitor used for myeloproliferative neoplasms, presents a moderate risk (1-10%) of HBV reactivation.
  • Current evidence lacks prospective randomized data to definitively recommend universal HBV prophylaxis in these patients.
