Kidney Transplant II: Surgical Procedure

38
Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living...

Summary

This study introduces a new sutureless partial nephrectomy technique using the Renal-Sutureless-Device (RSD). Initial results show it is a feasible and safe approach for kidney cancer surgery, preserving renal function.

Area of Science:

  • Urology
  • Surgical Innovation
  • Nephrology

Background:

  • Partial nephrectomy (PN) is crucial for preserving renal function.
  • Warm ischemia time (WIT) is a key modifiable surgical factor.
  • Traditional renorrhaphy for hemostasis can increase WIT and complications.

Purpose of the Study:

  • To evaluate the initial surgical experience with a novel sutureless partial nephrectomy technique.
  • To assess the feasibility and safety of the Renal-Sutureless-Device (RSD) in PN.
  • To report outcomes of PN performed without renorrhaphy using the RSD.

Main Methods:

  • A prospective study of 10 patients with cT1a-b cN0M0 renal cell carcinoma.
  • Surgical technique involved sutureless PN using the developed RSD.
  • Data collected included presurgical, intraoperative, postoperative variables, and functional results.

Main Results:

  • PN was successfully performed using RSD without renorrhaphy in all cases.
  • Median WIT was 12.5 minutes (IQR 10-15) in the 4 cases requiring renal artery clamping.
  • No complications, transfusions, or significant changes in renal function or blood counts were observed; 90% achieved disease-free margins.

Conclusions:

  • Initial experience indicates sutureless PN with the RSD device is feasible and safe.
  • The technique shows promise for simplifying PN and potentially reducing complications.
  • Further research is warranted to confirm the long-term clinical benefits of this innovative approach.
