Background:

  • Abnormal lung volumes, specifically air trapping, identify smokers at risk for developing spirometric chronic obstructive pulmonary disease (COPD) and adverse outcomes.
  • The evolution of lung volumes in early COPD as airflow obstruction develops is not well understood.

Purpose of the Study:

  • To investigate how lung volumes change during the development of spirometric COPD.
  • To analyze cross-sectional distributions and longitudinal changes in lung volumes across the airflow obstruction spectrum.

Main Methods:

  • Utilized pulmonary function data from U.S. Department of Veterans Affairs electronic health records (n=71,356).
  • Analyzed lung volumes from computed tomography scans in the COPD Genetic Epidemiology (COPDGene®) study (n=7969) and SPIROMICS (n=2552) cohorts.
  • Excluded patients with preserved ratio-impaired spirometry (PRISm).

Main Results:

  • Lung volume distributions and changes showed similar patterns across all three cohorts with worsening airflow obstruction.
  • Total lung capacity (TLC), vital capacity (VC), and inspiratory capacity (IC) exhibited nonlinear changes.
  • Patients with GOLD 1 (mild) COPD had larger TLC, VC, and IC than GOLD 0 or GOLD 2 (moderate) COPD. Baseline higher TLC and VC predicted progression to GOLD 1, while lower values predicted progression to GOLD 2.

Conclusions:

  • Lung volumes (TLC, VC) in COPD display biphasic distributions and nonlinear changes with increasing obstruction.
  • Lung volumes can identify GOLD 0 patients at higher risk for accelerated spirometric COPD progression.
