Is gastric emptying abnormal in duodenal ulcer?
This study found no evidence of abnormal gastric emptying in duodenal ulcer patients before or after healing. Gastric emptying rates were similar to controls, challenging the rapid emptying theory in duodenal ulcers.
Area of Science:
- Gastroenterology
- Digestive Physiology
Background:
- Duodenal ulcer disease is often associated with theories of abnormal gastric emptying.
- Previous research has suggested rapid gastric emptying as a potential contributing factor.
Purpose of the Study:
- To investigate gastric emptying abnormalities in duodenal ulcer patients.
- To determine if any abnormalities persist after ulcer healing.
Main Methods:
- Scintigraphic gastric emptying measurements were used.
- 16 duodenal ulcer patients, 12 pernicious anemia patients, and 12 controls were studied.
- Measurements were taken before, during, and after cimetidine therapy for ulcer patients.
Main Results:
- No significant difference in gastric emptying rate or pattern was found in duodenal ulcer patients before and after cimetidine treatment compared to controls.
- Solid food emptying was faster during cimetidine treatment.
- A tendency towards reduced differentiation between solid and liquid gastric emptying was observed in treated ulcer patients, similar to pernicious anemia patients.
Conclusions:
- The study questions the established belief that rapid gastric emptying is pathogenetic in duodenal ulcer disease.
- Gastric emptying of solids may be influenced by gastric secretion volume.
- No intrinsic gastric emptying abnormality was detected in duodenal ulcer patients.

Pathophysiology of Peptic Ulcer Disease: Injurious Factors
In the antrum region, G cells secrete the gastrin hormone that binds to gastrin-cholecystokinin-B (CCK2) receptors on parietal and enterochromaffin-like (ECL) cells in the fundic glands. Simultaneously, the vagus nerve releases acetylcholine, which binds to M3...

Peptic Ulcer Disease III: Clinical Manifestations and Diagnostic Studies
Few clinical manifestations differentiate gastric ulcers from duodenal ulcers. Distinctions in the location, timing, and pain relief are crucial for healthcare providers in differentiating between gastric and duodenal ulcers during clinical assessments.
