    Background:

    Observation:

    • Two sisters, both aged 66, presented with Barrett's oesophagus.
    • Endoscopic examination revealed remarkably similar lesion severity and extent in both individuals.
    • Both patients also had a hiatus hernia and low lower oesophageal sphincter pressures.

    Findings:

    • The striking similarity in clinical presentation and manometric findings between the sisters suggests a potential hereditary predisposition.
    • Low lower oesophageal sphincter pressure is a common finding in gastroesophageal reflux disease, a precursor to Barrett's oesophagus.

    Implications:

    • These findings support the hypothesis that familial factors may play a significant role in the development of Barrett's oesophagus.
    • Further research into genetic mechanisms could identify individuals at higher risk and inform preventative strategies.
