Background:

Macular buckling surgery is an effective treatment for myopic traction maculopathy.
Its use in the United States is limited due to the unavailability of commercial buckling elements.

Purpose of the Study:

To describe a novel technique for creating a macular buckle using accessible materials.
To address the limitations of current macular buckling procedures.

Main Methods:

A novel macular sling technique was developed using a 41 band for globe anchoring and a 240 band for posterior orientation.
A grooved sponge was guided under the macula to achieve a customizable tamponade effect.
The technique was applied to a patient with recurrent, complex tractional retinal detachment.

The macular sling successfully resolved the recurrent retinal detachment.
Visual acuity returned to the patient's preoperative baseline.
A significant hyperopic shift was the only adverse effect noted.
