Substance Use Disorders Affecting Sleep01:24

Substance Use Disorders Affecting Sleep

199
Substance use disorders involve a pattern of using drugs more extensively than intended and continuing use despite harmful consequences. This includes legal substances like alcohol and nicotine, as well as illegal drugs. These disorders often involve both physical and psychological dependence, reflecting compulsive use of substances that significantly alter thoughts, feelings, and behaviors, contributing to a major public health issue.

Drug Abuse and Addiction: Pharmacological Phenomena

578
Drug dependence, abuse, and addiction are complex phenomena that can precipitate various abnormal states. Physical dependence refers to a state of pharmacological adaptation to a drug. This adaptation often results in tolerance—a reduced response to the drug after repeated administrations. When the drug use is abruptly stopped, withdrawal symptoms occur due to the body's need to readjust from the pharmacologically induced imbalance. However, tolerance and withdrawal symptoms do not...

Pathophysiology of Diabetes01:20

Pathophysiology of Diabetes

1.0K
Diabetes mellitus is a chronic metabolic disorder characterized by hyperglycemia. The four categories of diabetes are type 1 diabetes, type 2 diabetes, other specific types of diabetes, and gestational diabetes.
Type 1 diabetes is characterized by autoimmune-mediated destruction of pancreatic β cells, with environmental factors potentially triggering this process in genetically susceptible individuals. Despite many not having a family history, certain genes increase susceptibility,...

Teratogenicity01:07

Teratogenicity

2.5K
The ability of a drug to produce structural deformations and functional abnormalities in the developing embryo or the fetus is called teratogenicity, and the drug producing this effect is known as a teratogen. Teratogenic effects include stillbirth, miscarriage, intrauterine growth restriction, and neurocognitive delay. A teratogen may affect the embryo at different stages of development, which is important in determining the type and extent of the damage. During blastocyst formation, the early...

CNS Depressants: Alcohol and Nicotine

300
Ethanol, a clear colorless alcohol, has been consumed by humans for millennia, but its effects on the body are far from benign. At lower doses, it induces decreased inhibitions and loquaciousness, leading to its social appeal. However, it can cause severe consequences at higher doses, such as coma and respiratory depression, due to its zero-order elimination kinetics. Chronic ethanol abuse wreaks havoc on multiple organ systems, particularly the CNS and the liver. Abrupt cessation of ethanol...

Drug Dependence01:17

Drug Dependence

1.1K
Medications are typically administered to achieve therapeutic effects. Some drugs can modify an individual's mood and perception, frequently resulting in various enjoyable experiences. However, this can result in drug dependency, a condition marked by continuous drug use despite potential negative consequences. Drug dependency primarily falls into two categories: psychological and physical dependence. Psychological dependence occurs when the pleasurable feelings induced by the drug...

Summary

Healthcare professionals lack confidence in managing perinatal substance abuse (PSA). Increased training and support, like drug liaison midwives, are crucial to improve care and reduce stigma for these high-risk pregnancies.

Area of Science:

  • Obstetrics and Gynecology
  • Neonatal Care
  • Public Health

Background:

  • Perinatal substance abuse (PSA) poses significant risks, including prematurity, low birth weight, and neonatal abstinence syndrome.
  • Effective care pathways and optimized staff/patient education are essential for managing high-risk pregnancies affected by PSA.
  • Understanding healthcare professionals' (HCPs) knowledge and attitudes is key to enhancing care and reducing stigma associated with PSA.

Purpose of the Study:

  • To explore the knowledge and attitudes of healthcare professionals regarding perinatal substance abuse (PSA).
  • To identify specific knowledge gaps among HCPs in managing PSA.
  • To inform strategies for improving care and reducing stigma for pregnant individuals with PSA.

Main Methods:

  • A cross-sectional study was conducted.
  • Questionnaires were administered to 172 healthcare professionals (HCPs) in a tertiary maternity unit.
  • Data collected focused on confidence, knowledge of referral pathways, and attitudes towards PSA.

Main Results:

  • A majority of HCPs lacked confidence in both antenatal (75.6%) and postnatal (67.5%) management of PSA.
  • Over half of HCPs (53.5%) were unaware of the correct referral pathway, and 32% did not know when to make a TUSLA referral.
  • Most HCPs (96.5%) desired further training, and 94.8% supported the introduction of a drug liaison midwife.

Conclusions:

  • Significant knowledge gaps and lack of confidence exist among HCPs regarding PSA management.
  • There is an urgent need for enhanced staff training, drug liaison midwives, and dedicated clinics to address PSA.
  • Implementing these measures is critical for improving care quality and reducing stigma for individuals and families affected by PSA.
