Background:

  • Trichilemmal cysts (TCs), also known as pilar cysts, are uncommon scrotal conditions.
  • Malignant transformation of TCs is rare, and multiple TCs in the scrotum are exceptionally unusual.

Purpose of the Study:

  • To report the first case of multiple scrotal TCs in Pakistan.
  • To discuss the surgical management of scrotal TCs and review existing literature.

Main Methods:

  • A 60-year-old male patient presented with scrotal swelling and multiple small scrotal skin lesions.
  • Diagnosis of TCs was confirmed, and the patient underwent hernia repair followed by scrotoplasty for cyst excision and scrotal reconstruction.

  • The patient experienced resolution of discomfort and achieved satisfactory cosmetic results post-scrotoplasty.
