Observation:

  • A 21-year-old male with APL experienced varied PICC tip location diagnoses.
  • Echocardiography in left lateral recumbent position suggested right atrial placement.
  • Trans-catheter echocardiography with shoulder abduction and chest CT in supine position indicated tip at cavo-atrial junction and superior vena cava, respectively.

Findings:

  • Discrepancies in PICC tip location were observed between ultrasound and CT scans.
  • Patient positioning during imaging significantly influenced the perceived tip location.
  • Contradictory findings underscore the dynamic nature of PICC tip position relative to patient posture.

Implications:

  • Standardized imaging protocols may need to account for patient posture.
  • Assessing PICC tip location in multiple, extreme daily postures is recommended.
  • This approach can enhance the safety and effectiveness of PICC utilization in oncology patients.
