Background:

The Medicare Quality Payment Program aims to improve healthcare quality and value.
Developing accurate cost measures is crucial for performance evaluation.

Purpose of the Study:

To develop and assess an episode-based cost measure for cataract surgery within the Medicare Quality Payment Program.
To evaluate the performance of this measure against nonselective cost assessments.

Main Methods:

A claims-based analysis of Medicare cataract surgery episodes (June 2016-May 2017).
Defined episodes from 60 days pre-surgery to 90 days post-surgery, including related services.
Calculated episode costs, clinician-dependent variation, and costs associated with complications.

Main Results:

Over 583,000 cataract surgery episodes were analyzed.
The episode-based measure showed higher clinician-dependent cost variation (94%) than nonselective measures (39%).
Episodes with complications incurred significantly higher costs ($3738 vs. $2276).

Conclusions:

The episode-based cost measure demonstrates superior performance in reflecting cost distribution, clinician influence, and quality alignment.
Ongoing maintenance and refinement are essential for the measure's clinical validity and reliability.
