  • Extrapelvic endometriosis, particularly sciatic nerve involvement, is uncommon and can lead to significant neurological deficits.

Observation:

  • A 29-year-old woman presented with progressive right hip and buttock pain radiating to the leg, causing gait disturbances.
  • Clinical and imaging findings suggested sciatic nerve endometriosis, later confirmed by histopathology.

Findings:

  • Surgical intervention involved transgluteal sciatic nerve excision and laparoscopic pelvic endometriosis excision.
  • A multidisciplinary team, including gynecologists, orthopedic surgeons, and neurosurgeons, managed the case.

Implications:

  • Cyclical sciatica symptoms warrant suspicion of sciatic nerve endometriosis.
  • Magnetic resonance imaging is the preferred diagnostic modality.
  • Multidisciplinary collaboration is vital for successful management of complex endometriosis cases.
