  • An 11-year-old girl experienced recurrent RLQ pain without significant appendiceal inflammation or swelling.
  • Repeated small ascites findings prompted an exploratory laparoscopy.

Findings:

  • Laparoscopy revealed a non-inflamed appendix with a middle segment atresia (cord-like narrowing).
  • Appendectomy was performed, and the patient remained asymptomatic during a 46-month follow-up.

Implications:

  • Appendiceal atresia should be considered in the differential diagnosis for pediatric recurrent RLQ pain of unknown etiology.
  • Diagnostic laparoscopy is a valuable tool for identifying rare causes of abdominal pain.
  • Early diagnosis and surgical intervention can lead to favorable long-term outcomes.
