    Ischemic optic neuropathy, a vision disorder, is identified as a new potential complication of Lyme disease. This rare manifestation may stem from direct spirochete invasion or immune responses.

    Background:

    • Lyme disease is a tick-borne illness caused by Borrelia burgdorferi.
    • Neurologic manifestations are common but varied.
    • Ischemic optic neuropathy (ION) is a known cause of vision loss, typically associated with vascular risk factors.

    Observation:

    • A case study of a 53-year-old man with Lyme disease is presented.
    • The patient exhibited symptoms consistent with ischemic optic neuropathy.
    • This presentation represents a previously unreported neurologic complication of Lyme disease.

    Findings:

    • Ischemic optic neuropathy was diagnosed in the context of active Lyme disease.
    • The exact mechanism remains unclear, but possibilities include direct invasion by Borrelia burgdorferi.
    • Alternatively, an immune-mediated inflammatory response triggered by the infection could be responsible.

    Implications:

    • This finding expands the spectrum of known neurologic complications of Lyme disease.
    • Clinicians should consider Lyme disease in the differential diagnosis of ION, especially in endemic areas.
    • Further research is needed to elucidate the pathogenesis and guide treatment strategies for this rare complication.
