Insights

Congenital anomalies of the kidney and urinary tract (CAKUT) are common. Lower expression of anti-fibrotic hsa-miR-29c-3p in obstructive CAKUT may drive fibrosis, suggesting therapeutic potential for miRNAs.

Area of Science:

  • Pediatric Nephrology
  • Molecular Biology
  • Genetics

Background:

  • Congenital anomalies of the kidney and urinary tract (CAKUT) are frequent, with ureteral obstruction leading to hydronephrosis, renal fibrosis, and chronic kidney disease in children.
  • MicroRNAs (miRNAs) are implicated in CAKUT pathogenesis, particularly in fibrotic processes.

Purpose of the Study:

  • To construct a miRNA-gene interaction network for CAKUT to identify fibrosis-associated miRNAs.
  • To experimentally validate the expression of selected miRNAs in pediatric CAKUT patients with obstructive conditions.

Main Methods:

  • Bioinformatic analysis to build a miRNA-gene interaction network.
  • Prioritization of miRNAs associated with fibrotic processes.
  • Quantitative real-time PCR to measure miRNA expression in patient samples (obstructed ureters, vesicoureteral reflux) and controls.

Main Results:

  • Identified hsa-miR-101-3p, hsa-miR-101-5p, and hsa-miR-29c-3p as significantly associated with fibrosis.
  • Extracellular matrix-receptor interaction was the top enriched molecular pathway.
  • Experimentally confirmed expression of these three miRNAs in CAKUT patients.
  • Observed significantly lower expression of hsa-miR-29c-3p in obstructed CAKUT groups compared to controls.
  • Found positive correlations between hsa-miR-101-5p/hsa-miR-101-3p levels and a significant correlation between hsa-miR-101 (-3p/-5p) and hsa-miR-29c-3p in the obstructed group.

Conclusions:

  • The significant downregulation of anti-fibrotic hsa-miR-29c-3p in obstructive CAKUT may contribute to the activation of fibrotic genes.
  • miRNAs, particularly hsa-miR-29c-3p, represent promising candidates for therapeutic interventions in CAKUT-related fibrosis.
  • Further studies are needed to measure fibrotic markers, assess fibrosis extent, and evaluate the functional role of hsa-miR-29c.
