The BioFire® FilmArray® Pneumonia (PN) panel detects bacterial pneumonia pathogens effectively, especially with high bacterial loads. Combining the PN panel with the leukocyte esterase (LE) urine strip test aids interpretation, particularly for low bacterial counts.

Background:

  • Community-acquired pneumonia (CAP) diagnosis relies on identifying bacterial pathogens.
  • Conventional culture methods have limitations in speed and sensitivity.
  • Molecular diagnostic panels offer rapid detection but require careful interpretation.

Purpose of the Study:

  • To evaluate the BioFire® FilmArray® Pneumonia (PN) panel's performance against conventional cultures for bacterial pathogen detection in CAP.
  • To assess the utility of the leukocyte esterase (LE) urine strip test in interpreting PN panel results.

Main Methods:

  • Sputum specimens from 67 CAP patients were analyzed using the PN panel, conventional cultures, and the LE test concurrently.
  • Pathogen detection rates and concordance between the PN panel and culture were compared.
  • The influence of bacterial burden and LE test results on concordance was analyzed.

Main Results:

  • The PN panel detected pathogens at a higher rate (59.7%) than culture (37.3%).
  • High concordance (76.9%) was observed between the PN panel and culture with high bacterial burden (≥10^7 copies/mL).
  • The LE test positivity correlated with higher detection rates for both PN panel and culture, and improved concordance.

Conclusions:

  • The BioFire® FilmArray® PN panel demonstrates high performance in detecting bacterial pathogens in CAP.
  • Ancillary use of the LE urine strip test is beneficial for interpreting PN panel results, especially when bacterial loads are low.
  • The LE test can help differentiate true positives from potential false positives in PN panel results.
