Background:

  • Posterior cortical atrophy (PCA) and logopenic progressive aphasia (LPA) involve distinct neurodegenerative patterns.
  • Longitudinal white matter tract degeneration in PCA and LPA remains understudied.

Purpose of the Study:

  • To investigate longitudinal white matter degeneration in PCA and LPA.
  • To identify phenotype-specific cross-sectional and longitudinal diffusion tensor imaging (DTI) biomarkers for PCA and LPA.

Main Methods:

  • Recruited 25 PCA, 22 LPA, and 25 cognitively unimpaired (CU) individuals.
  • Utilized structural MRI with DTI, including a one-year follow-up.
  • Applied mixed-effects models and receiver operating characteristic (ROC) analysis to assess DTI metrics.

Main Results:

  • PCA and LPA displayed overlapping white matter degeneration in occipital, temporal, and parietal regions.
  • PCA showed degeneration in occipital and parietal white matter; LPA in temporal and inferior parietal white matter.
  • Inferior occipital white matter integrity best differentiated PCA from LPA (AUROC=0.82).

Conclusions:

  • Findings enhance understanding of white matter degeneration in PCA and LPA.
  • Diffusion tensor imaging (DTI) shows promise as a diagnostic biomarker for these conditions.
