Allergic bronchopulmonary aspergillosis (ABPA) is an underdiagnosed lung condition. Early diagnosis and treatment with corticosteroids and antifungals can lead to satisfactory clinical outcomes in patients with asthma.
Area of Science:
- Pulmonology
- Allergology
- Infectious Diseases
Background:
- Allergic bronchopulmonary aspergillosis (ABPA) is a lung disease often overlooked in asthma and cystic fibrosis patients.
- It stems from an allergic reaction to Aspergillus fumigatus antigens colonizing bronchial mucus.
Observation:
- A 73-year-old female with a 35-year history of uncontrolled asthma presented for evaluation.
- Clinical presentation included asthma exacerbations, peripheral blood eosinophilia, elevated IgE, and positive Aspergillus serology.
Findings:
- Diagnostic imaging revealed bronchiectasis with mucoid impaction, confirming ABPA.
- The patient exhibited symptoms consistent with an allergic response to Aspergillus fumigatus.
Implications:
- This case highlights the importance of considering ABPA in refractory asthma.
- Prompt diagnosis and combined corticosteroid and antifungal therapy yielded positive clinical results.
- Increased awareness and diagnostic vigilance for ABPA are crucial in managing complex respiratory conditions.

Asthma is classified as allergic and non-allergic. Allergens such as dust mites, pollen, and pet dander trigger allergic asthma, while factors like cold air, intense emotions, or exercise can induce non-allergic asthma.
