Background:

Disseminated Nocardia infections are rarely documented, particularly in immunocompetent individuals.
Brain abscesses caused by Nocardia species present a diagnostic and therapeutic challenge.

Observation:

A large Nocardia brain abscess was identified in an immunocompetent patient.
The patient underwent surgical aspiration of the abscess.

Findings:

The patient showed significant clinical improvement following aspiration and a one-year course of intravenous antibiotics.
Follow-up imaging indicated complete resolution of the Nocardia brain abscess.

Implications:

This case highlights the successful management of a rare Nocardia brain abscess in an immunocompetent host.
The findings support prolonged antibiotic therapy as a key component in treating Nocardia brain abscesses.
Further literature analysis on Nocardia brain abscess management is warranted.
