    Summary

    Chest CT scans can now diagnose thoracic lipomas, avoiding surgery. This study identified lipomas using specific CT criteria in asymptomatic patients, recommending follow-up for homogeneous lesions.

    Background:

    • Historically, diagnosing thoracic lipomas required invasive thoracotomy.
    • Advancements in medical imaging prompt re-evaluation of diagnostic methods.

    Purpose of the Study:

    • To assess the efficacy of chest computed tomography (CT) in diagnosing thoracic lipomas.
    • To establish CT criteria for identifying lipomas in the thorax.

    Main Methods:

    • Retrospective analysis of 4,000 chest CT scans.
    • Identification of six patients with lipomas based on specific criteria: pleural mass, homogeneous fat density (CT numbers -54 to -129), and exclusion of other fatty lesions.

    Main Results:

    • Lipomas were incidental findings in asymptomatic patients (mean age 64.3 years).
    • Four of the six patients were male; half were obese.
    • Lesions ranged from 2 to 4 cm, located along the chest wall.
    • No malignancies were observed in the studied lipomas.

    Conclusions:

    • Chest CT can reliably diagnose thoracic lipomas in asymptomatic patients meeting specific criteria.
    • Clinical and CT follow-up is recommended for patients with CT-diagnosed lipomas.
    • Inhomogeneous lesions warrant biopsy or resection to exclude malignancy.
