Background:

  • Psoas muscle area (PMA) is evaluated as a predictor of frailty and functional outcomes in trauma patients.
  • Previous research has not fully elucidated the role of PMA in predicting recovery trajectories.

Purpose of the Study:

  • To assess the predictive value of psoas muscle area (PMA) for functional recovery in trauma patients.
  • To determine if PMA correlates with physical component scores (PCS) over time post-injury.

Main Methods:

  • A cohort of 211 trauma patients underwent abdominal-pelvic CT scans.
  • Psoas muscle area (PMA) was quantified in mm² and Hounsfield units.
  • Physical functionality was assessed using the Veterans RAND 12-Item Health Survey (PCS) at baseline and 3, 6, and 12 months post-injury.
  • Statistical models were stratified by Injury Severity Score (ISS) and adjusted for age, sex, and baseline PCS.

Main Results:

  • For patients with an ISS <15, increased PMA was significantly associated with higher PCS at 3, 6, and 12 months post-injury (P < 0.05).
  • This association was not statistically significant for patients with an ISS ≥15 (P > 0.05).

Conclusions:

  • Larger psoas muscles are linked to better functional outcomes in trauma patients with mild to moderate injuries.
  • PMA serves as a valuable prognostic indicator for physical recovery in specific trauma populations.
