Background:

Monitored anesthesia care (MAC) is frequently used for deep brain stimulation (DBS) device battery replacement.

Advanced Parkinson's disease presents unique challenges in perioperative management.

Observation:

A 63-year-old woman with advanced Parkinson's disease underwent a DBS battery change under MAC.
The patient had a history of intraoperative pain, anxiety, and inability to communicate during previous MAC procedures.
This history resulted in post-traumatic stress disorder.

Findings:

The patient's prior negative experience with MAC underscored the need for tailored anesthetic approaches.
Effective communication strategies are paramount when MAC is selected for patients with specific anxieties or conditions.
Informed consent and discussion of patient expectations are critical preoperatively.

Implications:

This case emphasizes the importance of individualized patient assessment in anesthetic planning.
Proactive strategies for intraoperative communication can mitigate patient distress and improve satisfaction.
Anesthetic protocols should adapt to patient history and psychological factors for optimal perioperative care.
